Ratings, totals and percentages follow the blank standard format (max 261 marks). Recommendations come from a fixed rule library built from the 12-Aug-2026 recommendations, so the same ratings always give the same actions.
Each rating column in the uploaded file is matched to a parameter in the standard format by its question text. If the form wording changes, matching falls back to column order. Check this list after editing the form.
Priority rules: rating 0 → P2 Essential, rating 1 → P2 Major, rating 2 → P3 Minor, rating 3 → no action. Parameters marked safety become P1 Immediate when rated 0 or 1. Phases: P1 0–15 days, P2 15–30 days, P3 30–45 days.
